Palliative Care Nurse Practitioner

LTC is seeking dedicated Advanced Practice Providers - Palliative Care to join our team of Primary-Care Providers.
Job Structure
- This is an in-person position
- Understanding and ensuring compliance with palliative care policies and procedures for clinical care, safety, infection control, hazardous materials and risk management.
- Learning and understanding the National Hospice and Palliative Care Organizations definition of Palliative Care:
- Palliative care is patient and family-centered care that optimizes quality of life by anticipating, preventing, and treating suffering. Palliative care throughout the continuum of illness involves addressing physical, intellectual, emotional, social, and spiritual needs and to facilitate patient autonomy, access to information and choice.
- Care is provided and services are coordinated by an interdisciplinary team.
- Patients, families, palliative and non-palliative health care providers collaborate and communicate about care needs.
- Services are available concurrently with or independent of curative or life-prolonging care.
- Patient and family hopes for peace and dignity are supported throughout the course of illness, during the dying process, and after death
- Effectively identifying, evaluating and addressing palliative and chronic disease management issues, while administering quality patient care.
- Standard Monday-Friday 8a-5p workday
- Full-Time
- 40-45 hours per week
- Experience in Palliative Care is strongly preferred.
- Candidates must hold a valid and unrestricted APP medical license in Michigan.
- Candidates must hold a valid DEA license in the state of Michigan.
- Candidates must have completed a valid U.S. APP certification program
- Potential to exceed 125K per year.
- Sign On Bonus
- Productivity Based Compensation Model (No Cap)
- Clinical Standards and Quality Measures must be met.
- 15 PTO days annually
- $2,500 Annual CME allowance plus 3 PTO days to use for CME attendance
- Company paid subscription to “Up To Date”, both mobile and laptop
- Monthly phone stipend
- All medical licensure expenses reimbursed by the Practice
