We are currently seeking a PART TIME RN Minimum Data Set Coordinator for our Westminster, MD community. The Carroll Lutheran Village RN MDS Coordinator is a central part of our health care network team, who conduct and coordinate the development and completion of the resident assessment in accordance with the requirements of this State and the policies and goals of Carroll Lutheran Village.

Duties:

  • Assess and evaluate the quality of care being given to long-term care residents.
  • Perform the initial interview and assess new residents.
  • Develop a care plan that has long-term and short-term goals for improvement.
  • Conduct meetings with multidisciplinary treatment teams, caretakers, and families.
  • Reach out to specialists within the community to get help in achieving care plan goals.
  • Prepare and electronically transmit timely reports to the national Medicare and Medicaid database.
  • Ensure the accuracy of medical records and coding procedures is essential for a facility to receive proper reimbursement.
  • Act on behalf of CLV’s residents' rights to advocate and ensure that each resident is aware of his or her legal rights.



Qualifications:

  • Must be certified as a Resident Assessment Coordinator (RAC-CT)
  • 4+ years’ experience in an MDS coordinator role or related experience
  • Current and valid state Registered Nursing license
  • Experience in an HCC clinical environment
  • Deep medical terminology knowledge
  • Exceptional mathematical skills and ability to apply them to tasks such as analysis of variance and sampling theory
  • Excellent leadership skills
  • Demonstrable knowledge of state and federal regulations
  • Superb decision-making skills
  • Proficiency in MDS software preferred
  • Must possess a strong desire to deliver care in an empathetic, compassionate way
  • Experience assessing, trouble shooting and making sound recommendations in stressful situations
  • Excellent communication skills to listen and communicate with residents and teammates
  • Proficiency in MS Word and Outlook
  • Time management skills
  • Dependability and reliability
