Schedule:

Flexible (3, 4, or 5-day work week)
Full Time (30-40 hours weekly)
Day shift only
No weekends
Fixed schedule/no on-call

Key Responsibilities:

Must be able to function at a high level with little oversight to provide nursing care, training, and home support (as needed);
Works collaboratively with Dialyze Direct team members, skilled nursing home facility representatives and dedicated Nephrologists;
Collects and records relevant patient health data in the course of/pre/post hemodialysis treatment;
Develops and maintains a plan of care that describes interventions to attain expected outcomes;
Implements and records interventions to attain expected outcomes in EMR;
Performs laboratory collection, processing, and shipping procedures and reschedules missed or inadequate laboratory collections;
Perform clerical tasks in addition to patient care (i.e., filing, inventory management, admission documentation, creating patient charts, etc.);
Maintains a positive and respectful attitude to all patients and colleagues;
Occasional travel to other nearby locations; and
Medication administration and documentation.

Requirements:

BS/AA/diploma in nursing with at least 12 months of RN experience (3 months in hemodialysis) preferred
Valid RN license
BLS certification; and
Ability to lift up to 35 pounds is required

Compensation:

Salary Range: $32.00 - $55.00 per hour. **BASED ON EXPERIENCE.**
