The Housing Coordinator helps members navigate housing related Social Determinants of Health (SDoH)/Non-Medical Drivers of Health (NMDoH) needs. This role is responsible for working with assigned members, the Service Coordination team, stakeholders (both internal and external) and community partners to create a person-centered service plan to improve the members’ housing outcomes and overall well-being. The STAR+PLUS Housing Coordinator assists members with community and facility-based transitions as well as education around affordable housing and other service options.


JOB SPECIFICATIONS AND CORE COMPETENCIES


20% Completes assessments with members and/or Legal Authorized Representative (LAR) to obtain information regarding member status, support system, housing insecurities, and other needs for housing services. Educate Members on accessing housing options such as utility assistance and the proper channels for reporting housing discrimination.

20% Responsible for collaborating with the Service Coordination Team, Internal and/or External Stakeholders (i.e., Provider, Caregiver, Program Staff), and referrals to Community-Based Organizations (CBOs).

10% Knowledge of Section 811 Project Rental Assistance (PRA), HUD, Housing Authority, Affordable housing programs, homeless service agencies and other supportive housing programs.

10% Knowledge of policies and procedures that are supportive of members who have disabilities or are age 65 or older.

5% Actively participate in Interdisciplinary meetings (IDT) to contribute to the development of care and service plans.

5% Actively contributes to achievement of departmental goals, as identified in Department’s annual business plan, including specific departmental process improvement plans and other duties as assigned.


MINIMUM QUALIFICATIONS:


Education/Specialized Training/Licensure: Bachelor's degree in business administration, Healthcare Management, or related field.

Work Experience (Years and Area): 3 years’ experience in employment, housing (Section 811 Project Rental Assistance (PRA), HUD, Housing Authority, Affordable housing programs, homeless service agencies and other supportive housing programs), transitions, or social determinant programs geared towards supportive programs. Some exposure to office practices and procedures preferred.

Experience working with LTSS, chronic or complex condition populations preferred.

Must be able to work with projects requiring attention to detail. • Good organization, critical thinking, and critical listening skills. • Ability to communicate with a diverse group of people. • Ability to work independently and to follow instructions. • Ability to travel throughout Harris County and surrounding counties. • Ability to work hours outside of normal workday.
