Nurse Care Coordinator

A healthcare services company is seeking a skilled Nurse Care Coordinator to join their team.
The Nurse Care Coordinator is a Registered Professional Nurse who provides direct assessment, evaluation and teaching to meet the needs of individual and families within their home and communities. The Nurse Care Coordinator is experienced in nursing, with emphasis on community health education/experience.
Qualifications:
- Registered Professional nurse in the state of NY
- At least two years' experience as a Registered or practical Nurse with one year administrative/supervisory experience in community health
- Completion of a baccalaureate degree in nursing or an associate degree with at least 1-year supervisory experience
Responsibilities:
- Completes assessments of patient and family to determine home care needs at regular established timelines.
- Provides a complete physical assessment and history of current and previous illness(es).
- Regularly re-evaluates patient nursing needs and processes information to comply with all Federal, State and City regulations, as well as all agency Policies and Procedures.
- Develops the plan of care with patient and family; establish goals based on nursing diagnosis and incorporates therapeutic and preventive nursing actions.
- Includes the patient and family in the planning process and makes necessary revisions as patient status and needs change.
- Provides health care instructions to the patient/family as appropriate per assessment and plan.
- Instructs, supervises, and evaluates home health aide care provided as stipulated by law and regulation.
- Prefills medication boxes as per MD's order.
- Communicates with the physician regarding the patient's needs, plan of care and reports and changes in the patient's condition; obtains/receives physician's orders as required and prepares clinical notes.
- Submits all documentation on a timely basis.
- Identifies discharge planning needs as part of the plan of care development and implements prior to discharge of the patient.
- Investigates and reports all incidents and complaints as per agency policies.
- Ensures that arrangements for equipment and other necessary supplies and services are available.
- Communicates with Director of Patient Services, agency administrator and other health related persons after new patient admission, resumption of care, and whenever the patient status changes.
- Anticipates the needs of the patient and recommends changes to Doctor and family as appropriate.
- Assures that all logs are maintained as required by Agency.
- Provides coverage for any other agency site as so requested by management.
- Participates in on-call duties as defined by the on-call policy.
