DESCRIPTION
Purpose Statement
The job of Home Visitor is done for the purpose/s of providing quality, comprehensive, home-based child/family support services for pregnant women and infants/toddlers and their families; providing support, training and case management to assist parents in meeting the needs of their infants/toddlers; and enhancing parent‘s role as the primary influence on an infant/toddler‘s education and development.
 
This job reports to Manager, Early Learning Program

Key Peninsula, Fife, Bonney Lake/Sumner/Orting, Clover Park, and Eatonville
 
Essential Functions
  • Actively recruits and enrolls pregnant women, infants/toddlers, and families for the purpose of providing comprehensive home-based child/family support services and/or encouraging parent involvement and maintaining full enrollment.
  • Analyzes child assessment data and collaborates with parents for the purpose of developing individual school readiness goals in five developmental domains.
  • Assesses family, pregnant women and infant/toddler needs through regular comprehensive home visits under potentially dangerous conditions for the purpose of supporting parent/child interactions and school readiness goals.
  • Assists families in arranging for transportation to program activities for the purpose of ensuring participation.
  • Collaborates with Paraeducators or childcare providers for the purpose of providing guidance and support in their interactions with pregnant women, infants/toddlers, and families.
  • Conducts health and developmental screenings and completes on-going assessments in the family‘s home or preferred language for the purpose of identifying needs, assessing children‘s health and development, and developing individual learning plans.
  • Coordinates the development and implementation of socialization groups, parent engagement, and other program activities for the purpose of supporting individual school readiness goals, parent/child interactions and parenting skills.
  • Develops individualized plans in partnership with all enrolled families for the purpose of facilitating progress toward family goals.

  • Identifies family strengths, health care needs and social service needs for the purpose of providing referrals and support services to infants/toddlers and their families.
  • Maintains a variety of manual and electronic documents, files, and records for the purpose of data collection, recordkeeping/documentation and tracking services to pregnant women, infants/toddlers and families.
  • Participates in workshops, meetings, community events, etc. for the purpose of receiving and/or presenting information.
  • Participates as a team member and/or partner in interdisciplinary planning for pregnant women, infants/toddlers and families with community partners such as Maternity Support, Early Intervention (Birth to Three) providers, Child Protective Services, Department of Children and Family Services, Department of Social and Health Services, Department of Corrections, community mental health providers, etc. for the purpose of providing case management to assist parents in meeting the needs of their families.
  • Promotes physical well-being and community connections to agency providers for the purpose of supporting optimal growth and development.
  • Provides services for pregnant women (e.g., health/nutrition/mental health support and follow up, perinatal education, resources, etc.) for the purpose of delivering services to expectant families in conformance with established guidelines.
  • Recruits and trains parent volunteers for the purpose of strengthening community engagement.
  • Responds to inquiries from a variety of sources for the purpose of providing information and/or direction.
  • Reviews and updates immunization records for each infant/toddler for the purpose of ensuring infants/toddlers receive vaccinations on an age-appropriate schedule and/or compiling information for the annual immunization reporting.
  • Supports families in accessing Early Head Start programming in their home or preferred language for the purpose of ensuring culturally and linguistically responsive services.
 
Other Functions
  • Assist other personnel as may be required for the purpose of ensuring an efficient and effective work environment.

MINIMUM QUALIFICATIONS
  • Home Visitor Child Development Associate (CDA) credential, or Associate?s Degree or higher in Early Childhood Education (ECE) or related field with coursework equivalent to the Home Visitor CDA
  • Experience working with children age birth-5 and their families.

PREFERRED QUALIFICATIONS
  • Experience in Head Start/Early Head Start/ECEAP or other comprehensive child development/family support program serving low income families
  • Experience working with infants and/or toddlers
  • Experience working with pregnant and/or postpartum women
  • Experience providing home visits with pregnant women and/or families with children age birth-5
  • Experience working with refugee families, immigrant families, and/or families with limited English speaking abilities
  • Community outreach experience
  • Bilingual ability - Spanish preferred
  • Experience working with interpreters
