GENERAL SUMMARY:
The physical therapist is responsible for examining, evaluating, and developing a treatment plan for patients with functional limitations. PT's to provide consultation to healthcare providers regarding patient's continuum of care.

ESSENTIAL DUTIES

Direct patient care:
Provides a Physical Therapy evaluation, incorporating Patient treatment: evaluates, interprets findings; develops treatment and discharge plans; implements treatment, re-evaluates, educates patients and significant others; meets productivity

Documents:

evaluation results, treatment and discharge plans and daily treatment according to all applicable laws, rules, regulations, and professional standards of documentation; submits charges for services provided.


  • Maintains verbal, written and electronic communication and collaborates with interdisciplinary treatment team, referral sources, physicians, and 3rd party payors.

Participates in leadership functions:
community outreach, continuous quality improvement, marketing, cost containment, program development, PTA/AT/aide/student supervision; teaches staff, physicians and others.

Professional development:
Attends staff meetings; maintains and improves competence through self-study, attending in-services and continuing education programs. Clinical education to students.

  • Manage staff and department resources wisely. Supervise assigned support staff. Communicates appropriately with patient, caregiver, and family regarding treatment and discharge planning.
  • Provide documentation to the physicians regarding the patient initially, at discharge, and at appropriate intervals. Discusses changes to the prescribed program with the physician before initiation.
  • PT maintains good standing with the licensure/regulatory board and all applicable laws/rules/regulations.

STANDARD REQUIRMENTS

  • Exhibits excellent customer service skills and behaviors toward internal and external customers and co-workers. Promotes positive public relations with patients/residents, family members, guests, and others.
  • Supports and participates with all required compliance standards that may be department specific and/or identified by the organizations including in-service training, acceptable attendance, uniform and dress code.
  • Adheres to HIPAA requirements and maintains confidentiality of all data, including patient/resident, employee and operations information.
  • Supports and participates in a collaborative team oriented environment – cooperates and works together with all co-workers, plans and completes job duties, uses appropriate communications in sensitive and emotional situations and follows up as appropriate regarding reported complaints, problems and concerns.
  • Supports, cooperates with and demonstrates safe work practices and attitudes, follows safety rules – including universal precautions - reports and prevents/corrects unsafe conditions and behaviors, and participates in organizational and departmental safety programs.
  • Completes all required compliance standards that may be department specific and/or identified by the organization.
  • Maintains current licensure, registration and/or certification, as applicable, at all times.

STANDARD QUALIFICATIONS
This individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required.

  • Education / Training: Bachelor's Degree in Physical Therapy required. Masters or Doctorate Degree preferred.
  • Work Experience: two years minimal.
  • Written or verbal communication skills required. Word processing and computer skills required.
  • Michigan State License in Physical Therapy required. Preferred BLS certification.
  • Other Qualifications to include: Competent clinical and evaluation skills are required.

Ability to demonstrate strong customer service standards; excellent verbal and written communication; ability to work independently in a fast paced environment; knowledge of clinical pathology, evaluation and treatment techniques for assigned population.

Must be able to demonstrate basic computer proficiency for EMR documentation.
