*Responsibilities:*
* · Evaluate, plan, and administer treatment to residents and patients with temporary or long-term disabilities to relieve pain, restore or improve function, and promote healing
* · Assure all treatment is delivered following an established plan of care
* · Provide clinical support and instruct patients, families, and caregivers
* · Monitor patient response to treatment intervention
* · Complete required forms and documentation following company policy and state/federal regulations

Premier Genesee Nursing and Rehab is currently seeking an in-house FULL TIME Physical Therapist/PT to provide direct patient care services under the direction and supervision of a Physical Therapist. The Physical Therapy Assistant will be responsible for documenting patient treatments, changes in condition and other pertinent information as required.

PTA Duties Include:
* Provides skilled physical therapy services/interventions in accordance with physician orders under the supervision of a Registered Physical Therapist.
* Responds to requests for service by relaying information and referral to Registered Physical Therapist.
* Implements and modifies the patient’s physical therapy plan of treatment with RPT supervision
* Measuring & recording residents motor function, strength and muscle performance
* Monitors patient responses to treatments and modifies it during treatment sessions as necessary.
* Observe residents and write progress reports.
