Summary

Outpatient General Mental Health (GMH) Behavioral Health Interdisciplinary Program (BHIP) team-based care provides Veteran-centered, recovery-oriented, and evidence-based care. The incumbent in this position functions as a Coordinator for Veterans on a BHIP team and is responsible for coordination and continuity of care for Veterans receiving services throughout the Mental Health Service (MHS).

Duties

Duties to include but not limited to the following:
a. Assume responsibility for ensuring that the BHIP services comply with VHA and Medical Center policies, federal and state local regulations, and meets joint commission requirements.
b. Tracks and collects data on utilization of behavioral treatments and develops performance improvement initiatives.
c. Provides training, orientation, and guidance to other health professionals regarding the parameters of BHIP services.
d. Ensures single, comprehensive Veteran-focused treatment plans with measurable goals are develop by BHIP team members.
e. Establishes clear referral mechanisms and processes for other facility providers, including formalizing service agreements, where needed.
f. Assesses available information and analytical systems to evaluate and enhance the quality of service provided to patient sin the general mental health population. Assists in develop and implementing information systems to track service activities including visits, referral, and patient satisfaction. g. Trains new BHIP team members about BHIP beyond what orientation and supervisory information includes

The BHIP Coordinator will also provide the following clinical services:
a. Provides case management services to include internal and external referrals, resources, education, advocacy, and supportive services.
b. Provides screening, assessment, and consultation for Veteran's needing mental health treatment.
c. Develops individuals' treatment plans.
d. Administers tools for measurement-based care to measure and improve patient outcomes.
e. Provides individual, group, and family therapy.
f. Completes suicide risk evaluations utilizing assigned and shared templates in the Computerized Patient Record System (CPRS) on all patients with suicidal ideation and on any patient who record is flagged, High Risk for Suicide.
g. The incumbent will participate in on-call rotation.
h. Willingly accepts additional and special assignments that are necessary to meet program goals or customer needs.

Work Schedule: May vary depending on deployment and/or assigned facility needs
Travel: Up to 50% may be required.
