Position Summary:

 

This is a highly professional position responsible for identifying client alterations in health, assessing the client's responses to actual and potential health problems and prescribed treatments, and providing appropriate nursing interventions to return the client to baseline health with appropriate physician orders. This position requires the ability to build professional, empathic, caring and supportive relationships with youths, and to work as part of an interdisciplinary team, while ensuring efficient and quality services.  Services may sometimes be provided in home or community settings.

Minimum Qualifications:

Must be a Florida Licensed Practical Nurse or Registered Nurse with at least two years of experience. Preferred experience working with people who have substance use and/or co-occurring disorders. Must have working knowledge of CAT model, behavioral and treatment interventions, and ability to deliver health education.

Driver Requirement:

Must have a current Florida Driver's License with a clean driving record while meeting company insurance requirements

Essential Duties/Responsibilities:

1.Monitor all aspects of the medication regime for CAT clients.

2.Document the effectiveness, side effects and/or responses to medications for clients. 

3.Provide specialized services, i.e., medical assistance, substance abuse education, health and wellness education. 

4.Assist Psychiatrist with medication monitoring for clients. 

5.Provide vitals and other biometric screenings for clients.

6.Document work in the Electronic Medical Record.

7.Monitor the health and dental needs of the clients.

8.Provide other needed services to patients in the program. 

9.Act as a participating member of the interdisciplinary CAT.

10.Attend staffings as required. 

11.Consult with other medical personnel to ensure proper consumer follow through and growth.

Knowledge, Skills, Abilities:

Specific competencies required are identified on a department/program specific basis and identified in each employee's Competency Checklist.  The following are general and apply to all positions with this title.

Language Skills:

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.  Ability to write routine reports and correspondence.  Ability to speak effectively before groups of customers or employees of organization.

Mathematical Skills:

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.  Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.

Reasoning Ability:

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.  Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
