The Family Home Visiting division of Health and Human Services is seeking a bilingual candidate who is fluent in both English and Spanish to join our team as a Community Health Worker, Senior. This position will provide outreach, case management, and home visits for prenatal clients living in Hennepin County. This position will also provide teaching on prenatal topics and provide culturally appropriate health education, connect clients with resources, and refer them to community-based agencies.

Location and hours:
This position is hybrid and will be performed both on-site at the North Minneapolis Human Service Center at 1001 Plymouth Avenue North, Minneapolis, MN 55411, in the community providing home visits, and remote as job duties require. Work hours are Monday through Friday, 8 a.m. – 4:30 p.m. While this position is designated as hybrid, based on current requirements hires must reside in or relocate to Minnesota or Wisconsin.

New employees who are hired into remote or hybrid positions between January 2, 2022, to December 31, 2024, will receive $500  toward the cost of establishing consistent internet connectivity, payable upon completion of 6 months of employment.

In this position, you will: Conduct client outreach and provide family home visiting services to pregnant and postpartum families. Create an individualized client education plan and provide education and anticipatory guidance using a curriculum. Conduct screenings and connect clients to appropriate community resources. Ensure services are client-centered, culturally appropriate, and equity focused.
  Need to have: One of the following:Completion of two years or more of college in social/behavioral sciences or another field related to the area.Two years or more of experience working in a community outreach, clinical/healthcare, or public health setting. Ability to communicate fluently in Spanish, in addition to English, both verbally and in writing. A valid driver's license and daily access to reliable transportation to travel within and outside of the metro area to meet with clients in their residential settings and/or perform authorized off-site work duties.
  Nice to have:
Experience:Working in family home visiting services or community health.Working with local non-profit organizations and/or organizations that work with pregnant or post-partum families.Working with individuals from a variety of diverse cultural and lifestyle backgrounds. Using databases and performing data entry where accuracy is essential.Working within a team, organization, or community to reduce health disparities. Knowledge of community resources and social services available to clients. Ability to:Establish and maintain effective working relationships with staff, community agencies, and culturally/socioeconomically diverse clients.Communicate information to people with different levels of understanding and diverse backgrounds.Show patience, compassion, and empathy, and listen to the client's needs.Handle several different problems and assignments, simultaneously.Identify members/clients’ complaints and/or needs.
