Hennepin County Streets to Housing is a program with a comprehensive understanding of the needs of those who are experiencing unsheltered homelessness in Hennepin County. The program was designed to develop a stronger response to households that are occupying encampments, as well as provide service coordination within encampments or to individuals who are staying in places not meant for human habitation. We believe that the solution to homelessness is housing, and that everyone is house-able. Everyone’s situation is unique. This small, dedicated team will work through this complexity by conducting assessments of individuals experiencing unsheltered homelessness and brokering necessary services through a trauma-informed, person-centered approach to ensure that households are deeply supported to resolve their housing crises as quickly as possible and to find safe, appropriate alternatives to sleeping outdoors through shelter, nighttime drop ins, family reunification, and housing options.

Location and hours:

These positions will be performed on-site at various community sites and outdoors in the field, within Hennepin County. Work hours will typically be Monday to Friday between 8a.m.-5p.m.

In these positions, you will: 
Engage with people experiencing homelessness in a person-centered, trauma informed way on a regular basis.  Conduct needs assessments for each encampment or space for unsheltered homelessness. Conduct housing needs assessments for the people staying outside. Work with each household to identify a safe, appropriate alternative to sleeping outside (i.e. shelter). Broker services and support unsheltered households to access and connect to referrals, including providing transportation, emotional support, and necessarily follow up. Assist with connecting households to benefits for which they are eligible. Work closely with Healthcare for the Homeless, the Hennepin County Homeless to Housing teams, and community street outreach partners to coordinate services for identified households. Work with the homelessness response system(s), and ancillary systems including adult behavioral health and family, and youth systems, to be conversant in all the housing solutions that may exist or be available to reduce barriers and resolve housing crises. Meet with neighborhood and mutual aid groups to ensure coordination of mutual aid and volunteer activities within encampments. Work with the City of Minneapolis’ Homeless Coordinators to meet the encampment needs.  Track and record data in HMIS and other program information tracking systems using established workflows. Comply with client data privacy standards. Work with various stakeholders in the housing and homelessness community to clearly communicate the work and mission of the Streets to Housing initiative. Need to have: One of the following:Two years or more of responsible, paid social service experience providing direct client service.A two-year human services certificate.Two years or more of college coursework in the behavioral sciences. Nice to have:
At least 2 years experience in a relevant Human Services role, including people experiencing homelessness and experience working with people living with mental health and/or substance use disorder.  Experience working directly with individuals experiencing homelessness. An understanding of housing solutions/options both inside and outside of the Coordinated Entry System. Ability to work within and across systems to assist people with navigating the complexities of these systems to reduce barriers and resolve housing crises. Commitment to ending homelessness though a Housing First philosophy. Proven ability to maintain a non-judgmental attitude and a display of unconditional positive regard when working with extremely vulnerable persons experiencing homelessness Ability to maintain professional boundaries.  Ability to be extremely creative and have a “whatever it takes”, solution-focused attitude when working with challenging situations. Familiarity of Trauma Informed Care, Motivational Interviewing, and Assertive Engagement. Knowledge of HMIS, SSIS, MAXIS, Excel, Outlook, etc.  Must have valid MN driver’s license and be comfortable transporting.
