Duties

Inpatient Mental Health social worker provides the following duties: Collaborates with multidisciplinary treatment team consisting of psychiatrist, psychologist and nursing staff; develops and implements psychosocial assessments, treatment and discharge plans for psychiatric inpatients; refers patients with mental illness and substance abuse to the appropriate least restrictive level of care. Social worker contacts collaterals and coordinates meetings with family members and outpatient providers; provides recovery oriented groups; documents clinical encounters using the online medical record system. Social worker makes post discharge phone calls within 48-72 hours of discharge. The social worker provides coverage for the acute and sub-acute inpatient mental health units and will have a tour of duty that will include weekday and weekend hours.

Functions to be completed during tour of duty (weekday/weekend)
- Independently performs psychosocial assessments with a wide variety of individuals from various socioeconomic, cultural, ethnic, educational, and other diversified backgrounds. Position requires a basic knowledge of human development and behavior (physical and psychological), and the differential influences of the environment, society, and culture.
- Has the ability to work with patients and families who are experiencing a basic emotional, medical, and social problems utilizing individual, group, and family counseling skills. Has the ability to assess the psychosocial functioning and needs of patients and their family members, and to formulate and implement a treatment plan, identifying the patient's problems, strengths, weaknesses, coping skills, and assistance needed.
- Develops treatment plans in collaboration with the veteran, family and with the interdisciplinary treatment team. Incumbent possesses the skills to offer psychosocial interventions and measures outcomes to interventions.
- Is able to coordinate community-based services based upon veterans needs and eligibility, including information and referral to additional services from other VA programs, other government program and community agency programs. The social worker collaborates with other service providers in reassessing the veteran's needs. The social worker is responsible for educating the veteran and/or caregiver of the available services and assisting them in establishing the appropriate referrals based on the veteran's preference or that of his/her surrogate decision-maker.
- Independently identifies high-risk patients and provides case management services. Incumbent provides crisis intervention services, seeking to address the cause as well as the presenting complaint, coordinates family conferences and serves as liaison to family members. Has working knowledge and experience in use of medical and mental health diagnoses, disabilities and treatment procedures. This includes detox, acute, chronic and traumatic illness/injuries.
- The incumbent has a high level of skill in Motivational Interviewing and Cognitive Behavioral Therapy. The incumbent has proficiency in, assessing and treating the complicated, psychosocial problems that include chronic medical and mental illness, substance use, frail elderly, homelessness, inability to care for self, frequent hospitalizations or emergency department visits, suspected victims of abuse or neglect, terminal illness, catastrophic illness or injury, non-compliance with treatment plans, or need for guardian or conservator.
- The incumbent will make three attempts within seven days of discharge to complete a post-discharge follow-up phone call when indicated.
- The incumbent will educate veterans, family and staff about advanced directives and will assist veterans in completing advanced directive/living will.
- Has general knowledge of veterans' benefits and services relating to special programs, services connected compensation, non-service connected pension.
- The incumbent understands the intimidation of bureaucracy and will act as an advocate when it serves the best interest of the veteran/family. When appropriate and feasible, social worker will educate and encourage the veteran/family to advocate on own behalf, thus fostering a sense of independence and empowerment.
- The incumbent is able to meet the needs of customers while supporting VA mission and consistently communicates and treats customers (veterans, their representatives, visitors and all VA staff) in a courteous, tactful and respectful manner. Incumbent is able to provide the customer with consistent information according to established policies and procedures.

Work Schedule : : First Shift Tuesday-Saturday 8:00am-4:30pm, Second shift Sun-Thurs 8: 00am-430p or 7:30am-4:00pm
