Job SummaryWe are seeking a highly skilled and compassionate medical social worker to join our team in a part-time capacity. As a Hospice Social Worker, you will play a vital role in providing patient-centered care and support to individuals and their families.
Responsibilities- Assist patients and families with health-related financial, social, and emotional concerns.
- Provide social work services in accordance with the plan of care.
- Communicate changes in patient status to healthcare team members and care center staff.
- Educate patients, families, and team members on medical social work interventions and treatment plans.
- Perform care planning, transfer, and discharge functions.
- Identify risks or threats to patient health and implement interventions to reduce risks.
- Masters or doctorate degree from a school of social work, accredited by the Council on Social Work Education.
- One year of social work experience in a healthcare setting.
- Meets state and federal requirements for this position.
- Current CPR certification.
- Attractive pay rate of $40 per hour.
