The Opportunity:

We are hiring a PRN Field RN for our Hospital In Your Home Program.

Saint Luke's now delivers hospital-level care to patients in the comfort of their own home. We are proud to be the first in the region to offer this quality service to patients in the Kansas City metro region.

Hospital In Your Home patients benefit from a quieter environment, visitors anytime, and better sleep. Similar programs at industry-leading hospitals have experienced lower hospital readmission rates, higher patient satisfaction, and improved outcomes.

Hospital in Your Home is seeking a highly motivated, autonomous, compassionate, and experienced Registered Nurse who helps maintain our high programmatic standards. As a RN in this care setting, you will work on a multidisciplinary team (MDs, APPs, case managers, pharmacists, etc.) to ensure the patient and family's acute care needs are met. This is a team recognized for their teamwork and collaboration amongst all members of the care teams. RNs on this team support each other, provide knowledge sharing, and work together to improve patient outcomes and improve their nursing skills.

The Schedule:

This position covers shifts 7 days per week. This is a PRN position.

The Hospital in Your Home RN position for the Kansas City metro area is a key member of the patient care team and has the following responsibilities:

  • Provides in-home nursing services and patient care to Hospital in Your Home patients.
  • Completes assessment of patient's physical and psychosocial status and needs.
  • Responsible for working with other members of the health care team when indicated to ensure optimal patient outcomes.
  • Executes orders and notifies physician and Command Center team of patient needs and changes in condition.
  • Demonstrates proficiency with clinical competencies and participates in-service training and continuing education opportunities.
  • Provides education and counseling to patients, family members, and other caretakers to support patient's care plan.
  • Documents patient visits, interventions, assessments, educational activities, and support provided.
  • Abides by legal and system rules, policies, and procedures.
  • All other duties as assigned.
