The Role:

As a Field RN in our Hospital In Your Home program, you will play a vital role in delivering comprehensive care to patients. You will work collaboratively with a multidisciplinary team of physicians, advanced practice providers, case managers, pharmacists, and other healthcare professionals.

Your Responsibilities:

  • Provide in-home nursing services and patient care, ensuring the highest standards of quality and safety.
  • Conduct thorough assessments of patients' physical and psychosocial needs, developing individualized care plans.
  • Execute physician orders, monitor patient progress, and promptly communicate any changes in condition to the healthcare team.
  • Educate patients, families, and caregivers on their health conditions, treatment options, and self-care strategies.
  • Maintain accurate and detailed documentation of all patient interactions, interventions, and assessments.

Qualifications:

To be successful in this role, you will need:

  • A current Registered Nurse (RN) license in good standing.
  • One year of experience as a Registered Nurse.
  • Advanced Cardiac Life Support (ACLS) and Basic Life Support (BLS) certifications from the American Heart Association or Red Cross.
  • Paramedic certification in both Kansas and Missouri.
