COMMUNITY PROGRAM SUPERVISOR II (FORENSIC ASSERTIVE COMMUNITY TREATMENT)

This position is located at the Kerns Building in NE Portland. The schedule is typically Monday through Friday, 8:30 a.m. to 5:00 p.m.

The Program Supervisor II works as a member of an interdisciplinary team to identify and coordinate needed services in the community for individuals assigned to the Forensic Assertive Community Treatment (FACT) Team. This position involves participation and implementation of program goals and objectives to promote a strengths-based, client-centered, trauma-informed approach to assisting clients in achieving individual goals and reducing recidivism. The Program Supervisor II assures that program activities are administered in cooperation with other team members and as directed by the Program Manager and Clinical Director. A Program Supervisor provides general and specific direction to team members, is responsible for management of the program, and serves as a backup to those supervised in addition to case management activities.

REQUIREMENTS

A Masters Degree in a behavioral science field from an accredited college or university. Demonstrate the competencies to become credentialed as a Qualified Mental Health Professional (QMHP) as outlined in OARs.
A minimum of three years of clinical experience providing inpatient or outpatient services is preferred.
Contribute to the development of annual goals and objectives for the program, consistent with the strategic goals of Cascadia Health and Cascadia's contract with HealthShare and Multnomah Department of Community Justice.
Provide consultation and work in collaboration with community partners, including Multnomah Department of Community Justice and Portland Police Bureau.
Provide direct clinical services to FACT clients at least 50% of the time.
Assess client behavior and design appropriate service plans including the ability to diagnose mental health disorders and addictions. During the assessment, obtain information: a) to establish the clients behavioral health needs, b) criminogenic risk and needs and required level of cognitive behavioral interventions to mitigate risk factors, and c) direct interventions and a plan for linkage or reengagement in community resources.
Assist in the teaching of daily living skills. Teach and assist in medically necessary services such as grocery shopping, maintaining clean housing, utilizing public transportation, maintaining self-care, exploring employment opportunities, etc.
