This is an
OPEN CONTINUOUS ANNOUNCEMENT and will remain open until September 30, 2023.

The initial cut-off date for referral of eligible applications will be November 21, 2022, with subsequent cut-off dates every Monday.

Eligible applications received after that date will be referred at regular intervals or as additional vacancies occur on an as-needed basis until position(s) are filled.


The RN Nurse Navigator will be responsible for coordinating and supporting activities associated with Gastroenterology/Hepatology patients from point of suspicion through diagnosis, continuum of care, and surveillance.

The incumbent will provide oversight of patient populations diagnosed with Advanced Liver Disease, Hepatitis, Fatty Liver, Cirrhosis, Hepatocellular Carcinoma, Autoimmune Liver Disease, and other hepatic diseases in collaboration with the Chief of Gastroenterology and Doctor of Pharmacy (Pharm D).

The RN Nurse Navigator will work collaboratively on a multidisciplinary team to co-manage the patient population.

Accountable for utilizing critical thinking skills and sound judgment; Serve as an educator on disease processes; track patients with complex care needs to optimize treatments.

The RN Navigator will serve as a representative on medical center and service level committees. Incumbent may be required to travel to the seven CBOCs supporting outpatient clinics for patient care, training, and meetings.

Monday - Friday, 7:30 AM - 4:00 PM (subject to change based on agency needs)
