About the Job

We are seeking a skilled Nurse Navigator to join our General Surgery Oncology team at Ellis Fischel Cancer Center. As a vital member of our multi-disciplinary team, you will play a key role in improving patient care and outcomes.

Key Responsibilities
  • Coordinate care along the continuum for specified disease processes or populations.
  • Act as a resource for patients, families, nursing staff, and other healthcare professionals in your area of expertise.
  • Provide education on disease processes and individualized treatment plans to patients and caregivers.
  • Drive best practices around programmatic quality indicators and lead/participate in quality/performance improvement activities.
  • Create processes specific to disease areas of focus to ensure access to care is achieved in a timely manner.
  • Deliver transitional care per area of specialty to ensure appropriate levels of care and compliance with treatment plans.
Requirements

We are looking for a candidate with one of the following qualifications:

  • Bachelor of Science in Nursing degree and three (3) years of RN experience.
  • Five (5) years of RN experience.
  • One (1) year of RN experience and five (5) years of LPN care coordination navigator experience.
