We are currently seeking a skilled and motivated Licensed Practical Nurse (LPN) to join our healthcare team. The work will be at an SNF (Skilled Nursing Facility).

Licensed Practical Nurse (LPN) Position Details :

Type: Per Diem

Location: Bellevue, WA

Responsibilities:

Understand and adhere to established policies and procedures.

Ensure adherence to Universal Precautions for resident and staff safety.

Prepare residents' medications for short-term absences following state regulations.

Collaborate with the healthcare team to implement the nursing care plan for each patient.

Initiate preventive and rehabilitative nursing procedures as appropriate.

Observe signs and symptoms and report to the physician and RN, including reactions to treatments and changes in the patient's physical or emotional condition.

Teach and counsel patients and their families/significant others on nursing care needs and related issues for home care.

Participate in medical record audits as assigned.

Attend staff meetings, patient care conferences, and in-services as scheduled.

Read Daily Communication Logs for resident information.

Document any changes in resident status in Progress Notes and review them regularly.

Complete resident treatments as ordered and scheduled.

Assist with personal care and physical support as needed.

Performs other duties as assigned.

Other Skills or Qualifications:

Proficient computer skills in managing the electronic health record (EHR).

Ability to manage information electronically and transfer records appropriately.

Ability to work in an electronic environment.

Refined deductive reasoning skills for problem-solving during shifts.

Must have a valid driver's license or dependable access to a vehicle for business use.

Must be in good health and physically/mentally capable of performing assigned tasks.

Working knowledge of personal computing and word processing software, particularly Microsoft Windows.

Completion of appropriate competencies as required by the facility.

Requirements:

Licensure: Current unrestricted license to practice as a Licensed Practical Nurse (LPN) in WA state.

Active BLS (Basic Life Support) certification.

Preferred minimum of one (1) year of nursing experience.

Strong communication and interpersonal skills.

Ability to work well in a fast-paced SNF environment.

Two (2) positive work references

Successfully complete and pass a background check and drug test.
