Is responsible for the day-to-day operation of the Non-Institutional Extended Care (NIEC) Program and Veteran-Directed Care (VDC) program (including providing administrative, technical, budgetary monitoring and oversight necessary for accomplishing Home and Community Based Care coordination).

Determines VDC program goals, working independently and with the Geriatrics and Extended Care (GEC) management in the development of plans for the attainment of these goals.

Performs a wide range of services and provides technical assistance to state, local, and private agencies. Monitors activities and program reviews and develops and analyzes information within the VDC program.

Uses sound judgement in the interpretation of trends and patterns within the program and incorporates a broad spectrum of factors when making programmatic decisions or recommendations to GEC management, taking into account the effects on other VA and non-VA organizations/activities, economic impact, and other similar such considerations.

Performs work related to the review, monitoring, analysis, planning, and assessment of VDC, as well as exploring and implementing solutions from a quality improvement aspect.

Initiates consultation with the Associate Chief of Staff (ACOS) of GEC and GEC Program Manager, as well as, with other authoritative resources at the Medical Center such as Regional Counsel, and Fiscal Services in unusual and/or complex administrative and clinical problems.

Seeks guidance from Veterans Affairs Central Office (VACO) program office as needed.
Participates in scheduled meetings, including local, Veterans Integrated Services Network (VISN), and VACO regarding VDC program reports. Provides program reports to Medical Center through GEC Committee; provides program reports to VISN and VACO as guided. Develops and implements information systems to track service activities including visits, referrals, patient demographics, and Veteran satisfaction. Negotiates, facilitates, and completes basic ordering agreements/contracts for the VDC.

Develops, implements, monitors application of Veteran Affairs (VA) guidelines to ensure standards maintained promote health, safety, welfare, and satisfaction of the Veteran receiving community services.

Coordinates appropriate oversight to ensure compliance with all VA policies, including but not limited to monthly meetings with the VDC provider, six month and annual Veteran contacts, and patient satisfaction survey per VACO guidelines.

Assures optimal qualitative/quantitative clinical program services via interdisciplinary collaboration, program development, and effective utilization of VA/community resources to provide appropriate services and patient advocacy.

Serves as resource person in the Medical Center for questions regarding VDC.
