This position is responsible for the development, implementation, and facilitation of a Person-Centered-Plan process for individuals served. This includes monitoring active treatment in accordance with ICF/IDD rules and regulations and the facility's policies and procedures.

The Mishkon Division provides supports and services in a healthy and safe environment for individuals with intellectual/ developmental disabilities living in residential programs and in community settings. Our core values are to provide a healthy and safe environment that promotes person centered planning, community integration, and close collaboration with families and the community. This is all accomplished through an Orthodox Jewish lens, and with an understanding that staff, at all levels of the division, is the determining factor in achieving positive outcomes for our clients.

Qualifications
  • BA/BS in a Human Services related field.
  • Two years experience in working with individuals with developmental disabilities or related field.
  • Understanding of OPWDD and DOH regulations and standards.
  • Must have an understanding of Goal assessments and implementation.
Responsibilities:
  • Train staff in in the areas of social issues along with instruction goals/ valued outcomes, and various clinically required trainings as per state regulations.
  • Responsible for the development and implementation of individual programming for the individuals served together with the other members of the Interdisciplinary team.
  • Attend Psychiatric appointments and day program meetings for the individuals on their caseload.
  • Coordinating services and the delivery of each resident's individual plan of care.
  • Integrating the various aspects of the facility's program and clinical services.
  • Complete observations and monthly documentation as required.
