Overview:

The Family Resource Specialists conducts outreach, interviews parents and builds community relationships to support the programs ability to meet the required capacity and utilization indicators. Family Resource Specialists must have an ability to develop trusting relationships and knowledge of infant and child development. The FRS must also have a capacity for introspection and an ability to communicate awareness of self in relation to others.

Responsibilities:
  • Responsible for reviewing hospital or clinic records for the purpose of screening new families for services.
  • Performs street outreach to enhance program enrollment of high-risk families.
  • Interface with other agencies and participates in community initiatives to develop and maintain community relationships and identify families for enrollment.
  • Interview parents prenatally/ and or immediately after the birth of their child.
  • Provide support and advocate for access to community health and other related services.
  • Assist Family Support Specialists in team effort to strengthen parent-child relationships and Fatherhood engagement.
  • Attends all Hospital and HFNY trainings within the required timelines.
  • Provides documentation of all above activities in accordance with Hospital and HFNY requirements and timelines.
  • Additional duties as assigned.
Qualifications:

Education:

  • Minimum of a high school diploma or equivalent, college coursework preferred.

Experience:

  • Must have experience working with or providing services to children and families.

Knowledge and Skills:

  • Excellent communication and trust-building skills.
  • Ability to be non-judgmental, compassionate, empathic and insightful.
  • Good interviewing and engagement skills.
  • Knowledge of community resources. Bilingual a plus.
  • Ability to fully utilize BHMCs electronic record systems, equipment and other healthcare and billing systems relevant to this position.
  • Shows respect and sensitivity with individuals of differing cultural, racial, sexual or ethnic identities.
  • Must be available for evenings, some weekends, and occasional overnight travel.
  • Ability to work in the field at least 3-days per week, conducting in-home assessments and attending community initiatives.

Physical Requirements:

  • Position requires prolonged periods of standing, reaching, walking throughout the working day.
  • Position will be required to stoop, bend, lift, and carry items weighing up to 25 pounds.
