This Float RN Staff - Primary Care position is located in Primary Care Service at the Bennington, VT location. This position is full-time at 40 hours per week.

Included but are not limited to:
This position is located at the Bennington Community Based Outpatient Clinic (CBOC) with occasional travel to the Rutland CBOC.

Join a dynamic Primary Care team comprised of providers, RNs, LPNs, and NAs and ancillary services including pharmacy, mental health, case management, nutrition and others.

Responsibilities include but are not limited to chronic disease management, walk-in and telephone triage assessment, post hospital discharge follow up and communication and, immunization administration.

Monday - Friday (Days)

May be available on an Ad-Hoc basis at supervisor's discretion

Pay:
