Major duties include, but are not limited to:


Responsible for the assessment, identification, facilitation and referral to appropriate medical and psychiatric services to those Veterans identified as appropriate for the HUD-VASH program.

Evaluates the Veteran's situation, abilities, and capabilities, and arrives at a reasonable conclusion including an assessment of vulnerability and prioritization for admission.

Assesses at-risk factors and develops a preliminary plan, involving the Veteran and family or significant others, and performs an insightful assessment of serious and complicated cases involving psychiatric illness which may also include, catastrophic medical condition, dementia, traumatic brain injuries, and other high risks diagnoses.

As part of a comprehensive psychosocial assessment, the incumbent interviews the Veteran and their family members or significant others to establish facts about the Veteran's situation, presenting problems and their causes, and the impact of such problems on the Veterans functioning and health.

Reviews all data, subjective and objective and makes a clinical assessment, identifying needs and strengths.
Based on the psychosocial assessment, uses professional judgment and social work practice skills to make a psychosocial diagnosis.

Provides referral to VA medical and mental health inpatient and outpatient treatment and to other VA services including VA benefits, as well as community-based social services and other non-VA entitlement programs.

If appropriate, provides referrals or consults for other VA programs including community-based residential treatment.

Uses social work clinical training, insight, and experience to interpret data and to identify viable treatment options, including assessment for eligibility and clinical appropriateness of a HUD-VASH program admissions or subsequent discharge.

Assesses high risk factors, acuity, and need for services. Makes appropriate referrals to community-based agencies, CBOCs and Central Iowa VA Health Care System based services.

Has the ability to serve Veterans who tend to have frequent and severe crises, lack family or an adequate community support network, be poor at self-monitoring, those that frequently fail to comply with instructions and treatment, or have significant deficits in coping skills and require continuing professional psychosocial support.

Work Schedule

Monday-Friday, 7:30am to 4:00pm

Pay:
Competitive salary and regular salary increases.

Paid Time Off:
37-50 days of annual paid time offer per year days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year).

Parental Leave:


After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.


Child Care Subsidy:


After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.
