Duties include but are not limited to:
Manage homeless program consults, conducts assessments, and develops follow-up plans for Veterans experiencing homelessness.
Conduct in-person assessments with Veterans on inpatient units in order to provide information and referral to appropriate housing-related resources
Respond to Homeless Hotline calls and provide cross-coverage for the HCHV Coordinated Entry Specialist and HCHV Outreach Specialist(s).
Engage with community partners, developing networks of appropriate referral resources, and facilitating access to needed services.
Provide services to Veterans in community-based settings such as shelters and transitional housing, in conjunction with the HPACT providers.
