Job Summary and Qualifications

The Clinical Manager reports to the Director of Clinical Operations of St. David's Care Partners. The Clinical Manager oversees St. David's Care Partners clinical integration program including quality management and care coordination. This role will support network operations including but not limited to, monitoring of progress towards network goals, care gap management, practice transformation processes, transitions of care for identified populations, and care coordination to drive quality and cost outcomes for the attributed populations.

  • You will be responsible for leading and managing the day-to-day operational aspects of the quality and care management team.
  • You will supervise clinical staff including care managers, care coordinators, and performance improvement specialists.
  • You will collaborate in the development and preparation of network practice communications and meetings highlighting clinical opportunities.
  • You will be responsible for contracted care gap management including weekly review, progress reporting, practice report distribution, data collection and final submissions.
  • You will evaluate network clinical initiatives and processes for effectiveness, adjust as needed to achieve desired results within the network.
  • You will lead the clinical specialist team to manage practice workflows and processes to close gaps in care and outreach to patients for transitions of care. Has the ability to perform care coordination activities to assist as needed with gap closure and patient outreach.
  • You will partner with care coordinators to review attributed patients who benefit from care coordination and work with Director of Clinical Operations to effectively manage the high risk, high cost, and high utilizers.
  • You will develop and maintain a centralized deliverables library, streamlining availability of templates and resource materials.
  • You will create, prepare, and present weekly updates on team performance and recommendations for improvement.
  • You will monitor practice progress on an ongoing basis, including analysis of data and identifying opportunities for improvement and report findings to network team.
  • You will identify problematic issues, determine reasons and actions needed to resolve, and escalate to the Director of Clinical Operations for providers/practices who are difficult to engage, are not performing as expected, and for medically complex patients.
  • You will communicate clearly, regularly, and effectively with all levels and functions to create an atmosphere of mutual respect, openness, and encouragement.
  • You will maintain the confidentiality of the meetings attended.
What qualifications you will need:
  • Bachelor's degree required.
  • RN preferred
  • Supervisory experience required.
  • Proficiency – Advanced knowledge of MS Word, Excel, and Power Point.
  • Adaptability – Able to maintain efficiency when experiencing major changes in work tasks or the work environment; adjusts effectively to work within new network structures, processes, requirements, or cultures.
