Job Description

The Opportunity:

As a Home Hospice Social Worker, you are responsible for assessing the psychological, social, emotional, cultural, and financial needs that impact patients' health and recovery. Providing professional social work services utilizing supportive counseling and clinical knowledge in assessing patient and family needs during illness crisis intervention and post-discharge planning. Collaborating with the health care team in transition planning and addressing needs promptly. Participating in program planning and development to meet customer needs within the health care system and community.

Shift: Part-time, 24 hours/week on weekends.

The Work:

In this role, you will have various responsibilities serving residents in the community's skilled nursing center for rehabilitation. Some specific responsibilities include:
  • Understanding social and emotional factors underlying a resident's illness
  • Completing MDS's and developing care plans with the health care team
  • Helping patients adjust to illness effects
  • Referring patients and family members to community resources
