Job Title: Traveling Social Worker

We are seeking a skilled Traveling Social Worker to join our team at Focus Staff. As a Traveling Social Worker, you will provide discharge planning and psychosocial interventions to patients in need.

About the Job

This is a contract assignment for 13 weeks, with a 5x8 day shift. You will work with patients and families to assess their social and emotional needs, provide social casework, and develop plans to meet their goals.

Responsibilities
  • Conduct social histories and assessments to identify patient needs
  • Provide social casework to patients and families
  • Develop and implement plans to meet patient goals
  • Collaborate with healthcare providers to understand patient needs
  • Document services provided in accordance with regulations and standards
Requirements
  • LCSW credential required
  • 2+ years of experience in social work
  • Excellent communication and interpersonal skills
  • Ability to work independently and make decisions
