Overview

As a Home Health Aide (HHA), you'll be providing personal care and related services in the comfort of the client's home, following instructions from the RN Case Manager. Your responsibilities include assisting with personal hygiene, preparing meals, supporting client mobility, and promoting mental alertness. It's crucial to observe and report any changes in the client's condition to the supervisor and maintain a safe environment. Additionally, you'll be coordinating visits and collaborating with other healthcare agencies. Communication skills, adherence to guidelines, and equipment operation are vital aspects of this role.

Responsibilities
  • Follow the care plan for safe and competent client care
  • Assist with personal hygiene and maintain a safe environment
  • Prepare nutritious meals and support client mobility
  • Promote client independence according to the care plan
  • Engage clients in activities to enhance mental alertness
  • Offer emotional and psychological support
  • Observe and report any changes in client condition
  • Perform housekeeping tasks for a comfortable environment
  • Document progress notes promptly
  • Coordinate visit schedules efficiently
  • Collaborate with other healthcare personnel
  • Attend required in-service sessions
  • Communicate effectively with the RN Case Manager
  • Report any client condition changes promptly
  • Adhere to the Home Aide's scope of practice
Job Conditions / Requirements
  • Must have adequate hearing, eyesight, and physical dexterity
  • Occasionally lift up to 30 pounds and perform physical assessments
  • Effective verbal, written, and visual communication skills
  • Adherence to company guidelines and policies
EQUIPMENT OPERATION
  • Utilize BP cuff, thermometer, stethoscope, pulse oximeter
  • Use hand washing materials

Qualifications
  • CNA Certification required
  • High school diploma or equivalent
  • Completion of state-mandated training hours if applicable
  • Computer proficiency for progress note charting
  • Strong problem-solving and multitasking skills
  • Compassionate and caring attitude
  • Familiarity with client-based equipment
  • Pass a criminal background check
  • Professional appearance and good health status
  • Understanding of personal care basics, housekeeping, and meal preparation
