Job Summary and Qualifications

Serves as the key point of contact to coordinate care with the patient, family, physicians and caregivers. Develops, documents, and coordinates the care plan. Assesses the existence of any psychosocial barriers to the plan of care and facilitates referrals to the clinical social worker for interventions as appropriate. Serves as a true patient advocate and a clinical resource for staff and physicians. Collaboratively works with caregivers to facilitate patient centered care and ensure a smooth transition across the continuum of care to achieve optimal patient satisfaction and wellness, improve care outcomes and efficient utilization of health services.

What you will do in this role:

  • Continually identifies opportunities to improve care for individual patients as well as populations of patients. Identifies opportunities for organizational improvements. Systematically collects timely and accurate data to evaluate opportunities. Stays abreast of changing organizational, care coordination and clinical trends, regulatory matters and third party payer requirements related to clinical care, discharge planning, precertification and after care benefits.
  • Assesses, coordinates, facilitates, and communicates a focused patient centered plan of care in conjunction with the patient, family and caregivers so to execute a safe and efficient transition for assigned patient population.
  • Provides for clinical and therapeutic interventions and manages variance to the plan. Collaborates with the clinical team and medical providers to initiate the anticipated discharge plan for the patients and ensure progression of the plan.
  • Care coordination interventions related to measurable goals for reducing readmission rates, preventing duplication of services in efforts to optimize financial reimbursement.
  • Maintains knowledge and competencies related to care coordination and practices within their scope.
  • Advocates for the patient and the organization in appropriate resource utilization by incorporating evidenced-based screening criteria, regulatory and payer requirements.
What qualifications you will need:
  • Required Experience: Minimum of 3 years in nursing experience in adult clinical care. Clinical knowledge, IT skills, effective interpersonal communication, critical thinking, successful problem solving, ability to work autonomously, innovative thinking, resourceful at navigating community resources, team building and leadership.
  • Preferred Experience: Previous 5 years in nursing practice and 3 years care coordination. Community or ambulatory care coordination experience. Primary care clinical experience. Experience with inpatient discharge planning work. Motivational Interviewing and Intrinsic Health Coaching. Understanding and sensitivity regarding challenges facing low-income communities. Entry-level proficiency in the Spanish language preferred. Computer proficiency – electronic health record experience preferred. Ability to flourish in a team system.
  • Required Education: Bachelor of Science in Nursing OR enroll in a BSN or advanced degree program within 6 months AND obtain a BSN within 4 years of hire date into RN position. Previous signed BSN commitment letter OR grandfathering will be accepted between departments and Mission Health entities. Exemptions may be approved with CNO System Council approval.
  • Preferred Education: Master's in Nursing
  • Required License: Must have and maintain current licensure as registered nurse with the North Carolina Board of Nursing. eNLC (Enhanced Nurse Licensure Compact) license may apply, licensee should confirm with NCBON.
  • Required Certification: BLS
  • Preferred Certification:Level I: Maintains nationally recognized certification in Case Management, such as ACM, CCM or ANA certification) Level II: Maintains nationally recognized certification in Case Management, such as ACM, CCM or ANA certification).
