Job Summary

We are seeking a skilled Perinatal Nurse Navigator to join our team at Mission Hospital. As a Perinatal Nurse Navigator, you will play a critical role in supporting patients through complex healthcare environments, optimizing care and outcomes.

Key Responsibilities
  • Facilitate patient follow-up through coaching, information transfer, and communication to improve continuity of care and enhance medical management of complex diseases.
  • Guide patients and families through the healthcare system, assisting with access issues to expedite timeliness of care and treatment.
  • Build relationships with patients and physicians, coordinate the defined plan of care, and assist with appointments and resolving barriers.
  • Provide education and connect patients with community resources to ensure efficient, high-quality outcomes.
  • Collect and analyze data to track navigation program performance and identify opportunities for care practice improvement.
Requirements
  • Basic Cardiac Life Support (BLS) certification
  • Service Line Specialty Certification (preferred)
  • One to three years of clinical experience required
  • Three plus years of experience as a nurse navigator and/or patient educator in a service line (preferred)
Education and Licensure
  • Bachelor of Science in Nursing or enrollment in a BSN or advanced degree program with a BSN within 5 years of hire date into RN position
  • Current licensure as a registered nurse with the North Carolina Board of Nursing (eNLC license may apply)
