Internal Medicine/Pediatrics physician to provide comprehensive, coordinated care to children and young adults with medical complexity in our Complex Care and Transition Clinic.
Health services offered include a primary care medical home, longitudinal outpatient care management, multi-disciplinary care, collaboration with the palliative care team, and facilitation of the transition from pediatric- to adult-oriented health systems.

Training in Internal Medicine/Pediatrics is preferred. Pediatricians who are comfortable seeing patients up to 25 years old will be considered.
