BRIEF OVERVIEW

The Director of Transitional Care is responsible for the administrative and operational leadership for programs designed to support designated moderate and high-risk patients as they transition from the hospital. This position will advance strategies for integrating and optimizing transitions of care to identify and address any gaps in care early, facilitate care coordination and prevent readmissions. In addition, the Director develops collaborative relationships with SHC and SOM stakeholders and community partners, to ensure seamless transitions, the highest clinical quality, and a positive patient experience.

  • Integrates program services with the hospital's primary functions by supporting the organizational mission, priorities, and goals, and by delivering optimal quality health care services.
  • Plans, organizes, directs, evaluates, and coordinates services within the Aging Adult Services, Transitions of Care Program, and other programs for targeted populations with transition needs.
  • Develops and continually tests and refines, appropriate interventions ensuring care at the right time, and right place, and is high quality and cost-effective.
  • Manages the fiscal, strategic, and clinical operations of transitional care programs.
  • Develops, inspires, and maintains an effective, visible, and well-respected team by hiring, developing, coaching, evaluating, and managing the performance of team members and supporting post-acute transitions.
  • Collaborates with inpatient case management/social work leadership team to develop strategies to coordinate resources and processes to reduce the length of stay, ensure safe, timely, efficient transitions of care, throughput, and improved patient outcomes across the health system.
  • Collaborates with other SHC and SOM leadership to align and coordinate services for optimal patient care, seamless hand-offs, and avoidance of duplicative interventions
  • Provides leadership of and participation in the SHC program planning, task forces, projects, and committees.
  • Maintains accountability for program operations, measurable outcomes, and sharing outcomes with stakeholders
  • Interacts across the SHC continuum, including acute, ambulatory, and community partners to coordinate transitions of care.
  • Partners on safety, quality, and clinical effectiveness across the SHC continuum and ensures compliance with all regulatory requirements
  • Manages complex interdepartmental and interdisciplinary relationships.
  • Maintains license, certification, and continuing education as required.
  • Performs other duties and attends committees as requested.

EDUCATION

  • Master of Science degree in MS, MBA, MPH, Nursing, Healthcare Administration, or a work-related discipline/field from an accredited college or university
  • Bachelor of Science Nursing

EXPERIENCE

  • Five (5) to ten (10) years of clinical experience
  • Five (5) years of progressive leadership and management experience within the ambulatory, acute, or post-acute healthcare environment.
  • Expertise in case management, clinical integration, operational effectiveness, project management, and knowledge of regulatory and value-based care models.
  • Knowledge of EMR Interoperability and population health data management.
  • Experience leading large-scale change efforts and/or academic medicine
  • Experience managing projects relevant to the delivery of mission-critical organizational programs and services
  • Any combination of education and experience that would likely provide the required knowledge, skills, and abilities as well as possession of any required licenses or certifications is qualifying

REQUIRED KNOWLEDGE, SKILLS & ABILITIES

  • Ability to plan, establish, direct, and execute a transitional care strategy for SHC
  • Ability to develop, analyze, and drive processes to obtain quality outcomes and other measures of success
  • Ability to manage a population health database
  • Ability to lead care coordination activities for rising and high-risk clients within the assigned programs
  • Ability to foster collaborative relationships with all entities involved in transitional care
  • Ability to communicate effectively at all organizational levels and in situations requiring instructing, persuading, negotiating, conflict resolution, consulting, and advising, as well as flawless written communication
  • Ability to establish and maintain effective relationships with widely diverse groups, including individuals at all levels both within and outside the organization and gain their cooperation
  • Ability to plan, organize, motivate, mentor, direct, and evaluate the work of others
  • Ability to resolve conflicts and/or negotiate with others to achieve positive results; establish and maintain effective interpersonal relationships
  • Ability to teach, coach as well as provide leadership, and influence others
  • Ability to work independently and exercise sound judgment in interactions with physicians, payers, patients, and their families
  • Ability to develop project measures of success, analyze progress and evaluate outcomes.
  • Knowledge of regulatory requirements and value-based reimbursement models.
  • Knowledge of case management principles, processes, and their practical application preferred
  • Knowledge of clinical documentation requirements
  • Knowledge of clinical, quality, and administrative facets of the healthcare industry
  • Knowledge of principles of process improvement methodology, business and management strategic planning, leadership development, and education techniques

LICENSES AND CERTIFICATIONS

  • Registered Nurse (RN) – State of California Licensure
  • ACM Certified Case Management OR CCM Certified Case Manager Preferred

Base Pay Scale: Generally starting at $ $114.41 per hour

The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to, internal equity, experience, education, specialty and training. This pay scale is not a promise of a particular wage.
