A Brief Overview

The Director of Patient Safety is responsible for the direction and leadership of operational, programmatic, and efforts to support a culture of safety and the delivery of safe and highly reliable care across Stanford Health Care (SHC). This includes minimizing factors contributing to unintended adverse patient outcomes, coordinating, implementing, and overseeing the organizational patient safety program, and ensuring timely reporting of and follow-up/corrective action are taken in response to such events/incidents in accordance with the requirements of organizational policies, the Joint Commission, and other external regulatory and accrediting bodies. Responsible for the Stanford Health Care Patient Safety program in collaboration with the Patient Safety Officer and managing the Patient Safety staff. The Director reports to the Executive Director of Quality and Safety and works closely with SHC stakeholders and executive leadership and in dyadic partnership with the Patient Safety Officer.

What you will do

  • Directs managers and staff who carry out the functions of the Patient Safety program.
  • Mentors, supports, and develops staff who carry out the functions of the Patient Safety program.
  • Identifies, measures, and analyzes key performance indicators for patient safety within the organization.
  • Leads the roll-out of large-scale patient safety initiatives in alignment with the organization's strategic direction.
  • Directs the management of the event reporting system across the organization, encourages reporting of events, ensures the integrity of data, and that information is sufficient for organizational leadership to make a determination regarding improvement opportunities and regulatory reporting.
  • Plans and manages the analysis, summaries, and reports regarding patient safety activity and performance to organizational leadership.
  • Develops and maintains ongoing mechanisms for monitoring action plans and deficiencies and proactive processes for areas at risk with regular reporting to the organization's patient safety committee, QPSEC, board quality and service committee and other related committees.
  • Develops mechanisms for communication of patient safety related information across the organization. Disseminates information regarding and plans for adoption of "best practices" and process improvements in patient safety.
  • Identifies barriers to the implementation of patient safety initiatives, recommends and implements corrective measures, and develops and implements improvements to the organization's patient safety program.
  • Strategically aligns analysis and action planning for patient safety with peer review, quality council and other standing organizational and service/department committees.
  • Plans and dynamically manages implementation of annual patient safety risk assessment, workplan, data analysis and interventions of quality/patient safety consultants throughout the organization.
  • Plans and leads annual patient safety culture assessments as part of organizational planning and improvement efforts. Collaborates with operational leaders on the implementation of evidence-based practices for improved patient safety culture.
  • Activates the learning system by fostering a psychologically safe environment and a Just Culture.
  • Leads PSO activities and collaborations.
  • Leads the implementation of organizational safety educational activities.
  • Partners with stakeholders to design and produce the safest and best possible outcomes.
  • Addresses complex problems through profound knowledge, adaptive leadership, and system-thinking approach.
  • Serves as a member of the Patient Safety Committee, administratively manages this meeting, or assigns a delegate.

Education Qualifications

  • Master's degree in a healthcare-related field from an accredited college or university required

Experience Qualifications

  • Seven years of progressively responsible and directly related safety and quality and improvement experience required.Five (5) years of supervisory or management responsibility required.

Required Knowledge, Skills and Abilities

  • Demonstrated ability to provide leadership and influence others, including building effective working relationships and mediating and resolving complex problems/issues
  • Knowledge of performance improvement methodologies
  • Knowledge of principles and practices of organization, administration, fiscal and personnel management
  • Current working knowledge of Joint Commission regulatory standards
  • Demonstrated ability to manage, organize, prioritize, multi-task and adapt to changing priorities
  • Knowledge of statistical analysis and reporting practices pertaining to quality improvement and program evaluation
  • Knowledge of medical terminology and related levels of care and treatment
  • Knowledge of Midas review and entry: mortality, PSIs, incident/event reports, Guest Services and Risk Management referrals, complaints, referrals from all sources
  • Knowledge of risk management and quality improvement principles and issues regarding patient safety
  • Understanding of human factors methods such as task analysis, human reliability analysis, heuristic evaluations, proactive risk assessment, incident analysis, and usability testing and how these methods can be applied in healthcare settings.
  • Demonstrated ability to present data and outcomes to support the value of designs and/or solutions to reduce risks and improve quality of care.
  • Demonstrated ability of application of high-reliability principles in complex adaptive systems.

Licenses and Certifications

  • State board licensure in nursing, social work, physical therapy, respiratory, pharmacy, or profession of relevance preferred . or
  • CPHQ - Health Care Quality Certification preferred . or
  • CPPS - Certified Professional in Patient Safety preferred .
  • The Just Culture Certification preferred .

Base Pay Scale: Generally starting at $ $104.87 per hour
