General Description:


The Admissions Coordinator assumes leadership in communication with multiple disciplines, community hospitals, nursing homes, and other VA facilities that refer Veterans for admission to the PUGET SOUND CLCS and Respite.

The Admissions Coordinator is familiar with the referral and consult process and ensures all components of the referral are completed.

The Admissions Coordinator uses problem-solving strategies by evaluating Veteran care needs, including the unit managers, providers, and a screening team to facilitate active team decision making, as appropriate, to ensure timeliness and appropriateness of admissions.

The information is reviewed by a screening team led by the Admissions Coordinator and a chart review is completed by members of the screening team to identify Veteran's needs and to ensure the Veteran is medically and psychiatrically stable for admission.

The Admissions Coordinator is an advocate of quality care for Veterans and works collaboratively with the Nurse Managers, Unit Providers, Chief Nurse, Extended Care and Chief of Extended Care to make sure Veterans are placed in the appropriate setting based on their care needs.

The Admissions Coordinator keeps key staff advised of bed availability within the PUGET SOUND CLCS during the week to facilitate effective utilization of beds.

The Admissions Coordinator assists with making contact with the contracting social worker for qualifying Veterans the PUGET SOUND CLCS cannot accommodate and need contract placement.

The Admission Coordinator assures specialized equipment is available and ready for the Veteran prior to their arrival and works collaboratively with PUGET SOUND CLCS leadership to ensure staff have competencies needed for any treatment or procedures that are necessary to meet the Veteran's needs.

The Admissions Coordinator provides coverage for the Restorative Care Coordinator and, at times the Resident Assessment Coordinator.

Duties may include but are not limited to:
Provides oversight of all external and internal admission referrals to the Community Living Center (CLC).
Monitors admissions, discharges, and transfer movements.-Monitors current and prospective bed capacity for the CLC.
Compiles data for submission for process improvement activities.
Reports on CLC admissions quarterly to Nursing Leadership.
Serves as a resource expert for the CLC Admissions process.
Leads quality improvement projects that relate to the CLC admissions process, policies, procedures, and Memorandum of Understandings (MOUs).
Participates in daily huddles with Nurse Mangers, education activities, staff meetings, and other required meetings throughout the year.
Will provide direct patient care as directed by GEC/PCS Leadership during times of shortage/critical staffing

Included expectations to:
Deliver care appropriate to age, gender, and diverse cultural needs.
Foster least restrictive patient care environment and actively seeks alternative interventions to restraint.
Provide coaching and education to caregivers regarding least restrictive environment and restraint use.
Assess patient for fall risk, implements and evaluates preventative interventions including fall prevention devices.
Shares knowledge of advanced directive policy and procedure. Collaborates with other health professionals to assist patient/significant other with quality of life issues.
Can provide education and coaching to caregivers regarding skin care issues and implementation of preventative nursing care.

Will provide unit leadership in directing nursing care and applying nursing process to unit patient care issues; i.e., skin care, falls, FIM, etc.

Is able to administer medications and/or treatments accurately and safely.
