Responsibilities- Establish goals of care and develop treatment plans for patients with heart failure
- Collaborate with healthcare team members, including social workers, cardiac rehab staff, and palliative care teams
- Manage decompensated heart failure and refer patients to the emergency department as needed
- Provide education and support to patients and their families
- Participate in quality improvement initiatives and contribute to the development of best practices in cardiovascular care
- Masters of Science in Nursing
- Post-Bachelor's Nurse Practitioner Program
- Successful completion of a Certified Nurse Practitioner Exam through an accredited organization
- Licensed in the state of WA as an ARNP by the time of employment
- DEA, NPI, and prescriptive authority
- Current BLS and ACLS for Healthcare Providers certification by the American Heart Association
- ARNP experience in a heart failure program or two years of experience in a cardiovascular setting
