We are looking for a Family Medicine Nurse Practitioner to be part of our dedicated team, providing evidence-based primary healthcare. As the ideal candidate, you will collaborate with patients and our healthcare team to create tailored care plans, ensuring holistic care following the medical home model, and offering clinical guidance.

Salary Offered: $150,000.00

Main Responsibilities:

  • Deliver evidence-based primary care to a daily caseload of patients, emphasizing preventive measures, delivering effective treatment for both chronic and immediate healthcare needs, and providing disease and case management.
  • Collaborate closely with patients and our healthcare team, which includes an in-house Whole Health Coach, to develop comprehensive biopsychosocial care plans that cater to each patient's requirements while respecting their diverse backgrounds.
  • Ensure thorough care aligned with the medical home model, incorporating an in-depth health history assessment, relevant physical examinations, necessary diagnostic tests, and medication prescriptions, all conveniently available through our on-site lab and pharmacy services.
  • Evaluate the healthcare needs of the patient population and collaborate with team members to encourage engagement in annual whole health assessments, health coaching sessions, and other health promotion initiatives.
  • Provide clinical leadership by actively participating in team meetings, maintaining clear proactive communication channels, consistently prioritizing essential tasks, implementing process enhancements, and actively supporting organizational changes.
  • Participate in rotational duties with other healthcare providers in the area to ensure seamless coverage during after-hours.

Requirements:

  • Must hold NP or PA Board Certification with a background in primary care, family medicine, or internal medicine.
  • Current, valid, and unrestricted medical license as an ARNP or PA, including a valid DEA number.
  • Minimum of 3-5 years of experience in primary care.
  • At least 2 years of experience in case management focusing on health prevention and population health management.
  • Proficiency in EMR systems; experience with NextGen is preferable.
  • Experience in coding and documentation, particularly with Medicare Advantage and HCC.
