SynerGrx is currently seeking highly motivated and skilled individuals to join our 12-month Fellowship Program.
We have found that a true residency program is not tailoring to the specific needs of the niche of the pharmacy industry we are focusing on, which is chronic care management.
The fellowship will be designed to provide pharmacists with a more robust training path, ensuring that they can continue their goal of working in this capacity.
Our fellowship training program in chronic care management will be in a similar financial proposition to our current residency program.
Upon successful completion of the training, we will be bringing on board those who have completed the training at our normal pharmacist rates, dependent on position availability.
We will be informing individuals who are being brought on to the position 90 days prior to the end of their training.
We believe that this transition to a chronic care management fellowship will better meet the needs of our organization and the specific niche of the pharmacy industry we serve.

Responsibilities:
Provide direct pharmaceutical care to patients in ambulatory care settings Participate in medication safety and performance improvement initiatives Provide education to patients and healthcare professionals Prescribe medications, order laboratory and diagnostic tests, and place consults and referrals to other healthcare services

Our clinical pharmacists and pharmacy interns partner with local providers to optimize patient outcomes by providing medication adherence solutions, optimize medication regimens, and manage difficult-to-control chronic health conditions.
