Job Summary

We are seeking a highly skilled Clinical Specialist Pharmacist to join our Ambulatory Oncology team. As a key member of our interdisciplinary team, you will be responsible for providing expert clinical care to patients with complex medical needs.

Responsibilities
  • Monitor and manage medication therapy regimens to ensure optimal patient outcomes
  • Conduct daily patient profile reviews to identify potential medication-related issues
  • Develop and implement individualized treatment plans to address patient needs
  • Collaborate with healthcare teams to ensure seamless care coordination
  • Provide education and support to patients and families on medication management and disease state
Requirements
  • Graduate of an accredited school of pharmacy
  • ASHP-accredited Post Graduate Year 1 and 2 Residency or Fellowship in Ambulatory Oncology or equivalent experience
  • Relevant Pharmacy Specialty Board certification preferred
  • Active Georgia Pharmacist license or eligibility for licensure
  • Basic Life Support (BLS) Certification
