Key Responsibilities- Monitor and evaluate patient medication therapy regimens to ensure optimal outcomes
- Perform daily in-depth patient profile reviews to identify potential medication-related issues
- Develop and implement strategies to prevent and resolve medication-related problems
- Collaborate with healthcare teams to provide comprehensive patient care
- Provide education and training to pharmacy students, technicians, and pharmacists
- Graduate of an accredited school of pharmacy
- ASHP-accredited Post Graduate Year 1 and 2 Residency or Fellowship in Hematology or equivalent experience
- Active Georgia Pharmacist license or eligibility for licensure
- Basic Life Support (BLS) Certification
- Advanced Cardiac Life Support (ACLS) Certification (within 1 year)
- Relevant Pharmacy Specialty Board certification
