Key Responsibilities
  • Monitor and evaluate patient medication therapy regimens to ensure optimal outcomes
  • Perform daily in-depth patient profile reviews to identify potential medication-related issues
  • Develop and implement strategies to prevent and resolve medication-related problems
  • Collaborate with healthcare teams to provide comprehensive patient care
  • Provide education and training to pharmacy students, technicians, and pharmacists
Requirements
  • Graduate of an accredited school of pharmacy
  • ASHP-accredited Post Graduate Year 1 and 2 Residency or Fellowship in Hematology or equivalent experience
  • Active Georgia Pharmacist license or eligibility for licensure
  • Basic Life Support (BLS) Certification
  • Advanced Cardiac Life Support (ACLS) Certification (within 1 year)
Preferred Qualifications
  • Relevant Pharmacy Specialty Board certification
