Job Responsibilities

- Provide care management for a caseload of 100 members, focusing on children in foster care, adopted from foster care, or in extended foster care.

- Assess healthcare needs, refer to health plan providers, educate on benefits, and provide community resources to address social determinants of health.

- Perform home visits, group home/hospital visits, and attend community/agency meetings.

- Document all activities in an electronic health record, including outreach notes, assessments, and care plans.


Performance Expectations

- Compliance with care management policies and documentation standards.

- Monthly audits to ensure compliance.


First Day Overview

- The first day will involve setting up the computer, completing onboarding tasks, reviewing the training schedule, and meeting the training team. The onboarding process lasts four weeks.


Required Experience:

- Pediatric case management experience.

- Experience in connecting families/children to local resources for services like employment, housing, independent living, justice, foster care, etc.

- Preferred Experience:

- Health plan experience.

- Experience in foster care and/or adoption.

- Education: Bachelor's Degree required.

- Software Skills: Proficiency in Microsoft Office (Outlook, Word, Excel).


Interview Process

- Panel interviews with two interviewers, each lasting 30 minutes.

- Preference for an all-day interview blitz from 9 AM to 5 PM with a one-hour lunch break.


Internal/External Collaboration

- Internal: Sunshine Health Care Management Department, Utilization Management, Medical Directors, Provider Engagement.


- External: Community-Based Care agencies, Managing Entities, DCF, Agency for Healthcare Administration, Providers.
