As the Case Manager, you’ll be responsible for managing a caseload of young adults who were formerly homeless/ low income.


The base pay range for this position is $22 to $24 per hour. The actual compensation offered to the successful candidate may vary from posted hiring range based on geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of the offer. This is a true temp-to-perm role; you can gain access to company benefits, paid time off, and more perks once you transition


Responsibilities:

  • Provide direct case management services both on-site and in the field. Services will encompass mental health assessments, developing service plans and assisting tenants.
  • Coordinate and document clinical case management and psychosocial services, while evaluating the overall effectiveness of provided case management services
  • Act as a liaison between tenants and community providers, facilitating groups focused on life skills, education, support, relapse prevention, and mental health counseling.
  • Make referrals to medical, mental health, and other external providers as needed.
  • Participate in community development efforts such as Community Meetings and support the residence in forming partnerships with other community providers.


Qualifications:

  • Bachelors degree in human services or related field
  • 1-2 years of prior experience working with young adults
