Job Title: Support Coordinator
Location: Bala-Cynwyd, PA
Duration: Permanent
Job Description: The Support Coordinator will support both children and adults (ages 3+) in the development of plans that assure the needs and wishes of the consumers are addressed and life opportunities expanded. Supports Coordinators assist in the choice of services and supports, coordinate the provision of services and supports, and monitor the delivery of services and supports. Responsibilities include weekly, monthly, or yearly on-site visits to the individuals' home to ensure all safety conditions are met, scheduling appointments with the individual, and on site travel around Philadelphia.
Position Requirements:
- Own form of reliable transportation
- Covid vaccinated
- Associates or Bachelors Degree with 12+ credits in Social Sciences
- Prior Human Services work experience (could be an internship)
Compensation:
$35,600 to $41,600 per year annual salary.
Exact compensation may vary based on several factors, including skills, experience, and education.
