Pay Grade: $77,563.20 to $93,412.80


POSITION SUMMARY

The Integrated Health Program Manager is responsible for management and oversight of grant-funded or contracted services, stakeholder relations, and informing program development for the integrated health program. The integrated health program advances the organization’s vision of Alameda County being the healthiest in the state by empowering AHC’s 8 member federally qualified health centers to support opportunities and address challenges around access to integrated healthcare services, with a focus on prevention and early intervention. This role will use community partnership, innovations, and data, to support the expansion and coordination of perinatal, pediatric, and adolescent behavioral health (BH) services. Essential functions of the position are:

The Integrated Health Program Manager can expect to collaborate cross-functionally with colleagues in the Development and Programs, External and Integrated Affairs, Healthcare Access, Data & Analytics, and Finance departments. External partners will include Care Coordinators, Directors and Clinical Mangers of Behavioral Health, Pediatric Leads, Maternal Health leads, and School Based Health Leads within member community health centers, and allied community-based organizations.

1. Management and Oversight of AHC’s grant-funded and contracted Care Coordinator programs, including Pediatric Care Coordinator, Integrated Behavioral Health Care Coordinator, and School-Based Care Coordinator services – 60%


a. Monitor performance of activities and services contracted to AHC member health centers, vendors, or other 3rd-party subcontractors to satisfy funder deliverables, including: i. Negotiating scopes of work with AHC member health centers, vendors, or other 3rd-party subcontractors to satisfy funder deliverables

ii. Coordinating with Development Manager, Director of Contracts, and when appropriate, Compliance Director, to ensure subcontractor agreements with AHC member health centers, vendors, or other 3rd-party subcontractors comply with AHC and CHCN contracting and compliance policies and procedures and are executed in a timely manner

iii. Ensuring timely receipt of quantitative and qualitative data from AHC member health centers, vendors, or other 3rd-party subcontractors to fulfill funder reporting requirements

iv. Reviewing and evaluating quality of performance by AHC member health centers, vendors, or other 3rd-party subcontractors to monitor progress toward expected outcomes; this may include by submitted reports, site visits, or other project-monitoring processes

v. Providing training, technical assistance, coaching, and facilitation of information-sharing about best practices to AHC member health centers, vendors, or other 3rd-party subcontractors to achieve expected outcomes

vi. Co-developing corrective action plans to achieve expected outcomes in the event of underperformance


b. Collaborate with supervisor and Development Manager to ensure timely submission of reports to funders

c. Pursue funding to sustain and evolve program implementation

d. Collaborate with Data and Analytics staff to evaluate impact of programming in line with organization’s Strategic Plan

e. Coordinate with Finance Department to ensure timely issuance of payments to subcontractors upon satisfactory performance of expected deliverables

f. Comply with AHC and CHCN’s Grant Administration policies and procedures, including participation in monthly Grant Managers meeting


2. Stakeholder Relations – 20%

a. Cultivate partnerships with Integrated Behavioral Health (IBH) and pediatric providers/staff in the clinics and school-based health centers, as well as appropriate staff of allied community-based organizations and/or Alameda County agencies to support successful delivery of care coordination activities in AHC member FQHCs

b. Facilitate peer network(s) and other cohort building activities for care coordinators and their supervisors at AHC member health centers on a monthly to quarterly basis to support achieving funded deliverables and community-informed evolution of the program

c. Share relevant information provided by Alameda Alliance for Health, CPCA, National Association of Community Health Centers, the Bureau of Primary Health Care at the Health Resources Services Agency with AHC member health centers

d. Identify and appropriately communicate Medi-Cal policy changes pertaining to integrated behavioral health and social determinants of health, specifically around prevention and early intervention


3. Inform program development and advocacy – 10%

a. Contribute to development of presentations or other collateral regarding mental health and substance use disorder programming in AHC member FQHCs to convey the impact of the integrated health program by effectively leveraging quantitative and qualitative data

b. Participate in appropriate peer networks facilitated by California Primary Care Association (CPCA) or other community collaboratives

c. Support and represent the AHC/CHCN integrated health team and FQHC IBH staff at local, state and county-wide meetings, policy forums and events

MINIMUM QUALIFICATIONS

• Excellent soft skills and desire to build trusting relationships

• Project Management: Ability to plan and organize work, managing timelines, budgets, and resources effectively for multi-year projects, as well as to prioritize tasksAnalytical acumen: Proficiency evaluating quantitative and qualitative data to inform data-driven evaluation program implementation

• Collaborative Workstyle: Demonstrated experience fostering collaboration with external partners of diverse backgrounds and experiences is critical.

• Demonstrated passion for addressing health disparities and healthcare advocacy

• Self-starter who has a desire to grow and develop professionally

• Experience facilitating groups and discussions

• Skilled at asking questions and getting to know complex systems and topics

• Sophisticated communication, both verbally and in writing

• Ability to work independently with minimal supervision and to collaborate effectively as part of a team

• Design thinking experience desired, such as human centered design approaches

• Proficient at Microsoft Office Suite

• Industry experience in health care preferred

• Lived experience as a Medi-Cal beneficiary and/or in mental / behavioral health care will be taken into consideration


SUCCESSFUL CANDIDATES WILL BRING

• A minimum of 2 years of experience in program management and grant oversight

• Expertise in integrated health in primary care settings, including an understanding of emerging trends in care coordination and primary care-delivered mental and behavioral health services

• Familiarity with federally qualified health centers and of the social and economic barriers that inhibit equitable health outcomes for low-income or other marginalized communities

• Compliance-minded orientation to ensure adherence with funder and contract requirements, including experience in budget development and management


EDUCATION, CERTIFICATES AND/OR LICENSES

• Bachelor’s degree required; Master’s preferred

• At least 5 years’ experience in healthcare or combination of relevant education and work experience
