The Medical Director works closely with the Chief Medical Officer to provide vision for and implementation of medical programs and services across Fora Health’s Departments. The Medical Director is an integral part of a collaborative team, which includes the Head of Psychiatric Services, Nurse Manager, and Medical Operations Supervisor. This team, in turn, collaborates with Fora-wide leadership, representing other programs in the continuum of care to support Fora Health’s mission of providing excellent care for those with substance use disorders.

The Medical Director will provide direct patient care in 3.7 level withdrawal management and medically managed residential programs. Additionally, the MD will direct expansion & integration of outpatient MAT services into Fora’s new Intensive Outpatient Program and will support that program clinically as needed. The MD will also collaborate with the Chief Medical Officer on development of an outpatient withdrawal management program. 

The Medical Director will join the Chief Medical Officer in nurturing a program culture that centers excellent medical care within a framework of loving kindness, harm reduction, and trauma informed care.

Expected allocation of time is approximately 60% administrative and 40% clinical. 


Essential Job Duties

·        Completing medical intakes on patients admitting to withdrawal management

·        Formulating treatment plans for those patients and communicating with nurses and case managers as necessary regarding implementation of medical orders and ongoing assessment of patients

·        Reviewing medical referrals to Medically Managed Residential Program and communicating with referents, including hospitals and outpatient settings.

·        Reviewing medical intakes on MMR admitting patients and consulting with nurse case manager on treatment plans.

·        Continuing expansion of MMR program to include patients on IV antibiotics and those newly dependent on insulin

·        Supporting the Outpatient WM nurse coordinator in outpatient intakes and being available for ongoing phone consultation with nurse coordinator and outreach peers in the field.

·        Leading weekly collaborative MAT Ops meeting

·        Providing clinical education to nurses

·        Sharing overnight phone call with Chief Medical Officer

·        Supervising and mentoring medical learners throughout their rotations at Fora

·        Providing education to patients about medications and treatment options.

·        Collaborating with Access Department to develop and expand relationships with community partners and pathways into Fora medical services.

·        Collaborating with Chief Medical Officer on development and implementation of new programs

·        Collaborating with Chief Medical Officer on writing and revision of policies & procedures, creating & maintaining EMR order sets, addressing grievances & incident reports.

·        Other duties as assigned

·        Negotiate conflicting demands of patients, staff, available resources, and applicable policies & laws.

·        Prioritize and manage the competing demands of their work effectively.


ORGANIZATIONAL RELATIONSHIPS

·        The Medical Director reports to the Chief Medical Officer.

·        The Medical Director works interdependently as a member of a program clinical team and works collaboratively with other agency staff.


Qualifications

Education and Experience:

·        Must be a licensed MD or DO

·        Must be board certified in Addiction Medicine; Fellowship training strongly preferred

·        Experience in prescribing buprenorphine required            

·        Experience in primary care, hospital medicine or urgent care required

·        Experience in withdrawal management strongly preferred

·        Supervising experience is a plus

Other Requirements

·        Verification of Sobriety, per Oregon Administrative Rules (OAR) standards

·        DHS Background Check Approval or alternate Criminal History Check agency approval

·        Successful completion of Drug Test upon hire

·        Documentation of Tuberculin test and/or evaluation with negative results or evidence of non-communicability

o  Conducts medical history review and physical assessment when necessary in a timely manner which does not compromise client health.

3.     Initiative: The extent to which the Medical Provider knows when to ask for help, prioritizes and efficiently and effectively performs duties.

4.     Integrity: The extent to which the Medical Provider performs job in a professional manner, demonstrating ethical behaviors and adhering to federal/state laws and agency policies and procedures.

5.     Reliability: The extent to which the Medical Nurse Practitioner accomplishes tasks with little or no follow up from the Supervisor.

o  Keeps the Supervisor informed of significant events that impact agency.

o  Completes projects in a timely manner.

o  Conducts clinical activities in a timely and appropriate manner.

o  Ensures that all clinical services provided are within the Services Model Framework.

o  Consistently employs Harm Reduction principles, Trauma-Informed care, and Motivational Enhancement techniques.

o  Completes all administrative and paperwork responsibilities in a timely and reliable manner.

o  Informs the Supervisor of any difficulties in performing regular duties
