Community Health Partner

Objective:
The Community Partner plays a crucial role in engaging patients into our care model to ensure that the organization can support their health needs. As a vital onsite role in communities where the patients live, the Community Partner will often be patients’ first point contact, setting the stage for a positive and trusting relationship. The Community Partner will be the most consistent onsite presence, allowing the Community Partner to work collaboratively with Providers and other Care Team members to address Patient Members’ care needs. The Community Partner will work closely with Patient Members to identify their social determinant of health (SDOH) needs and connect them to critical resources to address those needs. This role is crucial in contributing to the mission of enhancing healthcare accessibility and quality.
Essential Functions:
Build and maintain relationships with community members and enroll them into the clinical care model.
Support onsite delivery of care through facilitation of technology needs for virtual visits between patients and providers and through onsite support of care gap closure efforts
Conduct face-to-face screening with patients to identify their individual social needs (SDOH), coordinate resources to address those needs, and document note.
Work collaboratively with other members of the care team, including but not limited to providers, care navigators, social workers, and patient support associates.
Organize and facilitate community events, workshops, and meetings to foster collaboration and communication.
Build and maintain relationships with property stakeholders and partners to enhance community engagement and support.
Demonstrate empathy, active listening, and conflict resolution skills to address community members' concerns and needs effectively.
Maintain a positive and inclusive community culture by promoting diversity, equity, and inclusion principles in all interactions and activities.
Performs other duties as assigned.

Education & Experience Requirements:
Bachelor’s degree or equivalent, related work experience.
Experience in engaging and enrolling individuals into care delivery platforms or programs ideal.
Experience in using digital scheduling tools required.
Strong understanding of property relations and the ability to maintain positive relationships with community partners.
Experience working as a Community Health Worker and promoting health education and awareness in diverse communities, highly preferred.
Proven track record of actively participating in and contributing to community initiatives and projects.
Excellent communication and interpersonal skills to effectively engage with community members.
Strong organizational skills to coordinate and manage community and events.
Knowledge of medical terminology and experience working one-on-one with patients
Proficiency in basic computer tools and applications, such as email, Outlook calendar, Word, etc.
Passion for working in our community and making a positive impact on the health and well-being of residents.
