Schedule: Monday-Friday 8am-5pm
Contract: 3 month contract to hire
Pay Rate: $30-33/hour
Primary Responsibilities:
- Outreach and scheduling for patients post hospitalization
- Maintain accurate records of and and all outreaches
- Location and upload of discharge summaries
- Provide patient education on Medication Adherence
- Front Office Heavy
Required Qualifications:
- Active LVN license
- BLS Certification
- 2+ years of clinic experience
- HEDIS or Quality Audit experience preferred
- Bilingual (English/Spanish) preferred
