The Director of Critical Care Transitions & Education (CCLE) is a dual-impact leader who serves as both a hospital-facing clinical liaison and an internal educator focused on Critical Care Transitions (CCT). This role ensures every referral-to-admission process meets the highest safety and quality standards, while equipping nurses across the organization with the skills and confidence to deliver exceptional care at homeThis RN leader is the respected pediatric critical care expert within hospital systems—guiding families and medical teams through safe discharges and continuity of care. Internally, the Director oversees all CCT training, equipment education, and clinical readiness, coaching clinical leaders and field nurses to excel in critical care delivery.With equal parts empathy and resolve, this leader protects patients, families, and the Family First brand by ensuring transitions occur only when safe and appropriate—even when that means saying no.KEY RESPONSIBILITIESHospital Liaison & Critical Care Transitions (~50%)Serve as the primary clinical contact for NICU/PICU teams, case managers, and discharge planners.Guide referral-to-admission processes, ensuring patient readiness and compliance with Family First standards.Build and maintain strong hospital relationships through expertise, reliability, and professionalism.Act as the escalation point for transitions at risk, resolving issues and preventing breakdowns.Represent Family First at hospital meetings, caregiver presentations, and community events.Clinical Education, Training & Quality (~50%)Partner with VPCs to design, deliver, and oversee all CCT education, including equipment, devices, and competency assessments.Coordinate CCT admissions from referral through start of care, ensuring alignment with clinical leadership and offices.Partner with VPCs to ensure timely follow-up and reporting of changes in client condition.Track and analyze all CCT admissions to monitor performance and identify opportunities for improvement.Mentor Directors of Nursing (DONs), Clinical Managers (CMs), and field staff to build confidence and skill in critical care.Support broader education initiatives such as preceptor programs, accreditation preparation, and new market readiness.Evaluate training effectiveness through measurable outcomes in quality, safety, and patient satisfaction.Lead or support onboarding and continuing education for new offices, acquisitions, and growth initiatives.Leadership & CollaborationBridge clinical, business development, and operational teams to ensure aligned execution. Provide insights to the CNO and CDO from hospital relationships, referral patterns, and field education.Champion continuous improvement in referral-to-admission processes to enhance safety and efficiency.Model Family First’s culture of excellence—balancing high expectations with humility, empathy, and teamwork.Who Thrives in This RoleClinical Authority: Deep pediatric critical care knowledge that earns trust from hospital teams.Adult Learning Mindset: Skill in teaching, mentoring, and inspiring nurses from diverse backgrounds.Empathy & Resolve: Compassion for families, paired with the courage to uphold safety and integrity.Communication Excellence: Clear, persuasive, and professional communication across all levels.Calm Under Pressure: Steadiness in high-stakes, high-emotion situations.Organizational Mastery: Precision in managing multiple priorities, referrals, and trainings simultaneously.Bridge Building: Ability to navigate between clinical, business, and operational priorities with respect and alignment.ROLE QUALIFICATIONSEDUCATION & EXPERIENCEActive FL RN license (multi-state compact or eligibility)Significant pediatric critical care experience (NICU, PICU, or complex pediatric conditions)Proven leadership and clinical education experienceStrong relationship-building and communication skills with hospital and healthcare stakeholdersExcellent organizational and presentation skillsWillingness to travel frequently across Florida and TexasPREFERREDPediatric homecare or home health experienceExperience in quality improvement, accreditation, or new market launchesBSN or higher; advanced certifications in pediatrics, education, or critical careWORKING CONDITIONS/EQUIPMENT USETravelApproximately 75% of travel is required. Typically, WFH on Mondays; travel Tuesday–Friday for field, hospital, or training activities.Valid driver license with proof of insuranceMust be able to lift up to fifteen (15) poundsFrequent use of office machines to include telephone, computer, and printer
