The Clinical Supervisor has 24-hour responsibility for administration of day-to-day services for home care clients and Assisted Living residents; responds to tenant and resident emergencies and medical concerns. Operates as the Clinical Nurse Supervisor for Home Services and Assisted Living. Provides ongoing customer service to families and staff. Guides staff in compliance with Home Care and Assisted Living regulations. Has overall responsibility for the clinical management of the tenants/residents and the human resource management of the staff. Develops and administers all aspects of nursing services and other health care services provided by the campus and has direct supervision of Licensed Nurses and Home Health Aids.


Qualifications:


Education: Successful completion of an accredited Nursing program. Current Minnesota licensure as a registered nurse.


Experience:

  • 3+ years nursing experience
  • 2+ years of home health care experience including Assisted Living and Comprehensive Home Care requirements
  • 3+ years supervisory experience


Special Knowledge, Skills and Abilities:

  • Demonstrated working knowledge of Federal and State Home Health / Assisted Living rules and regulations; demonstrated ability to accurately interpret and communicate these rules and regulations
  • Effective time management, prioritization, and problem-solving skills.
  • Excellent written and verbal communication
  • Working knowledge of personal computers and related software
  • Excellent interpersonal skills
  • Ability to positively manage subordinates; demonstrated integrity and good judgment of initiative. Nurse Manager Home Care

2. Coordinates care for tenants and residents requesting home health or assisted living services.

A. Introduces home care services to clients, residents and families.

B. Performs initial assessment and reassessments of client and resident needs as required.

C. Plans, directs, and participates in delivery of professional services to clients.

D. Initiates and ensures each client/resident has a current Service Plan and provider orders.

E. Supervises care by home health aides and nurses.

F. Ensures an accurate and current client record by monitoring and or preparing clinical and progress notes or other required documentation.

G. Conducts follow-up assessments and supervisory visits, as required.

H. Monitors and ensures accurate service billing.

I. Maintains liaison with attending physician and other practitioners.

J. Notifies family in case of client/resident death.

K. Demonstrates knowledge of current health practices when performing observation and evaluations of client/resident care.


3. Assures home care program and Assisted Living is in compliance with applicable law and standards of practice.


Standards:

A. Maintains current knowledge of Home Care, Assisted Living and other related regulations.

B. Develops and administers appropriate home care and Assisted Living nursing related policies and procedures.

C. Ensures appropriate and timely education and training of staff.

D. Communicates regularly to department staff.

E. Monitors staff performance and addresses issues timely.

F. Ensures duties are performed safely.

G. Implements and maintains a Quality Management Plan, leads staff to achieve goals.


4. Conducts the wellness clinic and supervises.


Standards:

A. Conducts wellness clinic at scheduled times.

B. Performs nursing procedures within Nurse Practice Act parameters.


5. Responds to tenant emergency calls.


Standards:

A. Assesses emergency and tenant needs.

B. Performs medical intervention, as needed.


6. Initiates and or participates in resident conferences.


Standards:

A. Participates in care center/hospital conferences to assess needs for client/resident return.

B. Provides continuity between hospital, care center, and housing.


7. Meets with others to implement responsibilities.


Standards:

A. Conducts appropriate nursing staff meetings.

B. Facilitates nursing quality assurance meetings and develops and implements plans for assuring quality client/resident care.


8. Fulfills department personnel related requirements.


Standards:

A. Provides consultation and instruction to nursing staff regarding home health treatments.

B. Participates in hiring, orientation, productivity, and personnel evaluations.

C. Supervises, instructs, and assists with all client care and employee needs.

D. Ensures adequate staff education.


9. Maintains professional growth, knowledge, and development.

A. Actively participates in continuing education and in-services to maintain current licensure status and industry knowledge.

B. Attends appropriate geriatric nursing, nursing administration, regulatory, reimbursement, etc. related workshops, and seminars. Applies learned knowledge and educates others.

10. Follows and promotes adherence to State and Federal Regulations.

11. Performs other related duties as assigned.
